Changed url to create an account to sef link and now not working

We installed the component and all worked perfectly. But now while we still hVe the original link, the sh404ef link plus the 2 aliases work to get to the request an affiliate link but afte they fill out the info there is an error catchable favor error

    thanks. I can see the issue. the issue is actually that the form should not be showing directly.
    I think I can solve this easily, but I would need to change one line of code. can you provide FTP access?

    here is the link to create new affiliate account
    as you can see if u press submit form you will get error
    we are Using sef urls , also we are using htaccess to remove index.php
    all this is very important for us

    Please visit Cuckhoo.co and Open left menu, click on affiliate. Request new tracking code. Fill out the info.... Then you get error.

    We need the url of the affiliate location/menu item to be fully optimized for SEO. Can you please help us
